For external customers please fill out the Submission instruction (Tab 1) and Submitter Information (Tab 2) in Sample Submission Sheet External.
- After filling out Tab 1 and Tab 2 in Sample Submission Sheet, Contact us to schedule a consultation to fill the Sample Submission Form (Tab 3) in the sheet.
